View a Form

  3. Select a form to view

Manage Form Status

  1. Open a form request and select to mark as complete
  2. Optional: Select to undo
  3. Optional: Select or to mark read/unread
  4. Optional: Selectto delete the form request
  5. Optional: Select to add internal notes about the request

Reply via Text Message to a Form

  1. Select *Contact support if you do not have texting feature active.
  2. Multiple Locations - Select your location
  3. Multiple Text Numbers - Select your preferred "From" number

Find a Form Request

  1. Select and/or  to filter forms
  2. Select  to search by patient name, city, and form type

Restore Trashed Form Requests

  2. Select
